My child is ill and needs to have medication while at school. Where is the medication form that I need to complete before the school can consider giving the medication?
Please note that the school will not give your child medicine unless you complete and sign the medication form, and where appropriate Form AOM 1A which should be completed by a General Practitioner (GP). Click here for the medication form you need to complete.
I would like to make an application for leave of absence from school for my child. Is there a form that I need to complete to make an application?
It is School policy that only in exceptional circumstances will permission be given to take a child out of school during term time. We must take into account the effect this absence would have on the child’s education and ability to achieve. Failure to return to school on the specified date will result in those days being regarded as unauthorised absence and may result in the issue of a fixed penalty notice, prosecution in accordance with Section 434(6) of the Education Act 1996 and loss of placement at Blanche Nevile School for Deaf Children.
